Use this section to self-assess whether NDIS is the right pathway before starting an assessment.
- You are an active NDIS participant with an approved plan.
- You have a disability-related sleep, pressure care, transfer or positioning need documented by a treating clinician.
- You have an OT (or another suitable AT assessor) willing to complete the AT assessment.
- Your plan has funding available in Capital Supports (for mid or high-cost AT) or Core Supports (for low-cost AT).
- The request must align with at least one goal in the participant's current NDIS plan.
- Assessor credentialling must match the AT complexity band: low-cost under $1,500 usually needs no formal assessment; mid-cost $1,500 to $15,000 needs an appropriately skilled assessor; high-cost or complex above $15,000 needs a specialist AT assessor.
- The equipment must be reasonable and necessary, effective, beneficial, and represent value for money compared with alternatives.
- Trialled and considered alternatives should be documented, which is where the 7-night in-home trial evidence carries the most weight.
The evidence a referrer needs to submit
This is the reference section most OTs and case managers return to when they are writing a NDIS submission. Use it as a checklist.
Clinical evidence typically required
- Participant details and NDIS number.
- Statement of disability-related need and how it impacts sleep, pressure risk, transfers or positioning.
- Functional impact description including current sleep quality, skin integrity status, falls history and carer or family impact.
- A clear explanation of why the current sleep setup fails to meet the need.
- Alternatives trialled or considered, and the reason each was insufficient. This is where the Sleep Choice trial report is attached.
- Product specification sheet for the requested bed and mattress (dimensions, safe working load, cover, warranty, compliance).
- Itemised quote from an approved supplier.
- OT (or suitable AT assessor) assessment written at the correct complexity band for the item cost.
Format the evidence needs to be in
Most planners expect the NDIA AT assessment template (or your organisation's equivalent), with the product specification, itemised quote and Sleep Choice trial report attached as PDFs and signed by the assessing OT. Requests that bundle everything in a single, clearly labelled PDF pack tend to move faster than those submitted as loose attachments.
Common reasons applications get delayed or rejected
AT complexity band mismatch: assessment written at the wrong tier for the item cost.
Confirm item cost bracket before drafting. If cost sits close to a threshold ($1,500 or $15,000), request an updated quote so the tier is clear.
"Reasonable and necessary" not explicitly evidenced against a plan goal.
Quote the specific plan goal the request supports, and link the functional outcome of the trial (e.g. reduced night-time falls, improved skin integrity) back to that goal in plain language.
Alternatives not documented, so the request looks like a first choice rather than an evidence-based decision.
